FGF-19 belongs to a subfamily of the fibroblast growth factors (FGFs) called endocrine FGFs, which consists of FGF-19, FGF-21, and FGF-23. Unlike other subfamily of FGFs, FGF-19 has a low binding affinity to heparin sulfate and functions as a circulating hormone with effects on distant tissues. FGF-19 has reduced binding affinity to the FGF receptors and the interaction requires two necessary cofactors that are α-Klotho and β-Klotho. In the ileum, FGF-19 expression is initiated by bile acids-induced activation of farnesoid X receptor (FXR). In response to bile acid stimulation, FGF-19 is secreted and enters the portal venous circulation, which subsequently down-regulates hepatic bile acid synthesis through repression of the CYP7A1 gene encoding cholesterol 7-alpha-monooxygenase. The impaired regulation of bile acids by FGF-19 leads to bile acid diarrhea also known as idiopathic bile acid malabsorption. FGF-19 has also been demonstrated to regulate carbohydrate and lipid metabolism.
